I recently ordered some UHMW poly ethylene from Norva Plastics. I told them 
 what I needed, what it was going to be used for and they cut the piece to 
size  and shipped it to me very quickly and at a reasonable price.

You  can send them a sketch, the old part, or if you are really computer 
savvy, a  CAD file.  The will make the part from the material of your choosing 
 (UHMWPE?) and ship it to you.
